How to Fill Out This Sheet

Fill in the Account & Order Info at the top — this stays the same for the entire order.

Add a product section for each style. Please separate by gender — for example, use one product for Men's Singlet (include youth boys sizes) and a separate product for Women's Singlet (include girls sizes). This helps our factory identify and produce each item correctly.

Fill in the roster for each product — select a size, then enter the player name and number. If a product shares the same roster, use the "Copy Roster from Previous" button to save time.

When finished, click Download Excel to get a spreadsheet file you can share with your Sales Representative or Sales Coordinator.
